Website Privacy Policy
How Tyner Physical Therapy handles information collected through its public website
Effective October 1 2026
This website policy is not a substitute for any HIPAA Notice of Privacy Practices or other patient notice required by law.
This Website Privacy Policy explains how Tyner Physical Therapy ("Tyner PT," "we," "us," or "our") collects, uses, shares, and safeguards information when you visit tynerpt.com or any Tyner PT website that links to this policy (the "Site"). It applies to information collected through the public Site, including the contact form. It does not replace notices that may apply to medical records or other protected health information handled in connection with treatment, payment, or health care operations.
Please do not use the Site, ordinary email, or the general contact form to send detailed medical histories, diagnoses, test results, insurance identification numbers, Social Security numbers, payment-card information, or other sensitive health or financial information. If you are a patient, use the communication method Tyner PT directs you to use for clinical or billing information.
1. Information We Collect
Information You Provide
When you contact us through the Site, call us, or email us, we may collect information you choose to provide. The Site’s current contact form requests:
Your first and last name
Your email address and telephone number
The service or services in which you are interested
The contents of your message and any other information you submit
If you request an appointment, the information you send is an inquiry only. An appointment is not confirmed until Tyner PT responds and expressly confirms it.
Information Collected Automatically
When you use the Site, Tyner PT and service providers may automatically receive technical and usage information, such as:
Internet Protocol address and approximate location derived from it
Browser, device, operating system, and network information
Pages viewed, links selected, referring pages, and dates and times of visits
Cookie identifiers and information about how you interact with the Site
Information used to detect fraud, abuse, errors, and security incidents
Squarespace provides the Site’s hosting, forms, and built-in analytics. Squarespace may process technical browsing information for its own purposes as described in its privacy policy. The Site also uses Google reCAPTCHA to help prevent spam and automated submissions, and uses Adobe-hosted font services to display typefaces. Those providers may receive technical information such as your IP address, browser or device information, and the page you are viewing.
2. How We Use Information
We may use information collected through the Site to:
Respond to questions and appointment inquiries
Identify a suitable service, clinician, or location for an inquiry
Communicate about scheduling and administrative matters
Operate, maintain, secure, troubleshoot, and improve the Site
Understand Site traffic and how visitors use the Site
Prevent spam, fraud, misuse, and security incidents
Comply with law and establish, exercise, or defend legal rights
We will not use information submitted through the general contact form to make an automated decision about your eligibility for care. We do not intend to use sensitive health information submitted through the Site for advertising.
3. Cookies and Similar Technologies
The Site uses cookies and similar technologies. Necessary cookies support core functions such as navigation, security, form operation, and other features. Analytics and performance cookies may help us understand visits, traffic sources, page views, and Site performance. Third-party services, including Squarespace and Google reCAPTCHA, may also use cookies or comparable technologies when their services load.
Where required, the Site should present a cookie banner that allows visitors to accept, decline, or manage nonessential cookies. You may also control cookies through your browser settings. Blocking certain cookies may affect Site features. Any choices described in the cookie banner must match how the Site is actually configured.
4. How We Share Information
We may disclose information collected through the Site in the following circumstances:
Service providers. We may provide information to companies that support hosting, form processing, analytics, security, communications, and technical operations. The current Site uses Squarespace, Google reCAPTCHA, and Adobe font services.
Professional advisers. We may disclose information to attorneys, accountants, insurers, and consultants when reasonably necessary for their services.
Legal and safety reasons. We may disclose information when required by law or when reasonably necessary to protect rights, safety, property, patients, visitors, or the public.
Business changes. Information may be transferred as part of a merger, acquisition, financing, reorganization, sale of assets, or similar transaction, subject to applicable law.
With your direction or consent. We may share information when you ask or authorize us to do so.
Tyner PT does not sell information submitted through the Site for money. Before publishing this statement, Tyner PT should confirm that no advertising, retargeting, data-broker, or lead-generation service receives Site information in a manner treated as a sale or sharing under any applicable privacy law.
5. Health Information and HIPAA
This Website Privacy Policy governs the public Site. It is not Tyner PT’s HIPAA Notice of Privacy Practices. If Tyner PT is a covered entity under the Health Insurance Portability and Accountability Act and receives protected health information in connection with treatment, payment, or health care operations, separate HIPAA rules and notices may apply.
A public website form is not automatically a secure patient portal. Do not submit sensitive medical information through the general contact form unless Tyner PT has confirmed that the form and its vendors are approved for that purpose. Tyner PT should confirm its HIPAA status, its agreements with vendors that handle protected health information, and whether the Squarespace account and any connected storage are authorized for that use.
6. Data Retention
We retain information for as long as reasonably necessary for the purposes described in this policy, including responding to inquiries, maintaining appropriate business records, meeting legal or contractual duties, resolving disputes, and protecting the Site. Retention periods may vary according to the type of information and the reason it was collected. Tyner PT should document and follow a retention schedule for contact-form submissions and delete information that is no longer needed, subject to applicable recordkeeping obligations.
7. Data Security
We use administrative, technical, and physical measures intended to protect information against unauthorized access, loss, misuse, alteration, and disclosure. No website, email system, or method of electronic storage is completely secure. You should not send sensitive information through the Site or ordinary email. If you believe information you submitted through the Site has been compromised, contact us promptly.
8. Your Choices and Requests
Depending on where you live and the laws that apply, you may have rights concerning your personal information. These may include rights to request access, correction, deletion, or information about how personal information is used and disclosed. Some requests may be limited by legal exceptions, identity-verification requirements, or recordkeeping obligations.
To make a privacy request, contact us using the details in Section 13. We may ask for information reasonably necessary to verify your identity and authority. This section concerns personal information handled through the public Site. Requests for medical records or rights under HIPAA should be directed through the process stated in Tyner PT’s Notice of Privacy Practices, if applicable.
9. Children
The Site is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information online from children under 13 without appropriate authorization. A parent or legal guardian who believes a child has provided personal information through the Site may contact us to request review and deletion, subject to applicable law. Parents or guardians should submit appointment inquiries for minors.
10. External Links and Social Media
The Site may link to maps, social-media services, or other third-party websites. Those services operate under their own privacy practices, and Tyner PT is not responsible for their content or privacy practices. Review the third party’s policy before providing information. A link does not imply that Tyner PT controls or endorses the third party’s privacy practices.
11. Visitors Outside the United States
Tyner PT operates in New York. If you access the Site from outside the United States, information may be processed in the United States and in other countries where service providers operate. Privacy laws in those locations may differ from the laws where you live. Tyner PT should obtain legal advice before intentionally offering services to residents of jurisdictions with additional privacy requirements.
12. Changes to This Policy
We may update this policy to reflect changes to the Site, our practices, or legal requirements. The effective date at the top shows when the policy was last revised. Material changes should be posted prominently or communicated by another appropriate method when required by law.
